I spent a lot of time to detect the problem on your site and I added the CSS style below to components/com_rsappt_pro3/sv_apptpro.css file

select, textarea, input[type="text"], input[type="password"], input[type="datetime"], input[type="datetime-local"], input[type="date"], input[type="month"], input[type="time"], input[type="week"], input[type="number"], input[type="email"], input[type="url"], input[type="search"], input[type="tel"], input[type="color"], .inputbox {
    display: inherit;
}

Now, you can see the Appointment Booking Pro component is working as well on your site.
