Hi,
I have purchased the following template: (http://www.joomlart.com/demo/#ja_teline_v), to upgrade the old template, but it seems that there is no form in template purchased.
missing this: (Joomlart advanced custom module) how do I retrieve this form? thank you!

Are you missing the whole ACM module? If so simply go into the downloads section where you can download the individual components – http://www.joomlart.com/forums/downloads.php?do=cat&id=540
