Applying my solution will help you to solve the problem on your site.

Open templates/ja_simpli/css/template.css file
```
#content .contact h3 {
    display: none !important;
}
.form-horizontal .controls input, .form-horizontal .controls textarea{
    width: 300px !important;

```
