Looking for some advice on a form extension – if there is one like I am looking for. I am fairly new to Joomla and not really sure how to exactly ask my question here so that its easily understood.

I would like to have a form that when completed, would send the results back to the visitor and would offer advice or other information based on the choices they make in completing the form. In this case, they would select a statement that best describes them and the submitted for would be displayed on screen and would email them the results. Both the display and the screen would have snippets of advice after each one of the statements that they selected.

As an example, suppose the visitor was asked to choose their favorite color, and they chose blue. when they select blue, the form would return a script that told them the significance of the color blue. A copy of their submission would be emailed directly to them as well.

Its sort of like a quiz, except that they are not to be scored on their answers, just given advice on what they could do to improve their behaviour. I have checked out a number of “quiz maker scripts” but none are customizable enough to email the visitor back with suggestions/advice based on the color they selected

A kind of example can be found at the url below, except that I can’t determine what script they are running. If you answer all 22 questions on this site, a summary document is served up and it shows your answers as well as providing you with advice snippets below the selection you made for that question.
