Is it possible to change the two heading circled here http://prntscr.com/fb33l0 in this new contact form I have made? But ONLY in this contact form and not in others on the site. http://www.theplatinumlist.com/index.php/restaurant-sign-up

Hi Jim
Its default Joomla contact form, you can change the values of heading but it will be changed for all contact pages since the Joomla contact form is working from same files.
You can change the values from /root/language/en-GB/en-GB.com_contact.ini
