Dear Rockjoom,

– Plz go to module manager >> edit JA Quick Contact module >> change username to Full Name
– This module does not support phone number field, so you need to do more customization on the form layout of this module.

Please try to clean all cache. I checked this solution on our demo and it is working well.

JA Quick Contact module reads ‘Full Name’
default.php reads ‘Full Name’
ajax_layout.php reads ‘Full Name’

JA Quick Contact module set to ‘No Caching’
All Cache cleared

Still says “User name” on site
and “Official Inquiry” instead of ‘Subject’

What is the solution?

You can do that

Go Admin -> Modules Manager-> JA Quick Contact Module-> change it, see the screenshot

Remember clear cache from Admin area

I have checked your site I appreciate the hard work on your site, but with more and more customization you should hire a developer to carry out such work, customization support is out of JA support scope.
