I’ve identified the issue—it was related to the Layout settings of the template assigned to the Contact Us page on your website.

The template includes two layout styles: one specifically for the homepage, and another default layout for all other pages. However, the homepage layout was mistakenly assigned to the default template style. This layout doesn’t include the component block, which is responsible for displaying the main content area—hence, the page appeared empty.

I’ve reassigned the correct default layout, and the contact form is now displaying as expected.
