You can download attached file >> unzip and copy the 'edit.php' file to this directory:
templates/t4_blank (your T4 template)/local/html/layouts/t4/
If this path doesn't exist, pls create it.

I have added the edit.php file in the site and that has improved the page output. There are still module positions that are not displaying on the page.

saguaros The site is locked down fairly tightly. I'll need to know the IP address from where you will be accessing the site and the country so that I can tunnel you through my FW.

Pages with issues:

On the 'Edit Profile' page, there is not module assigned to sidebar-l or sidebar-r position so nothing showed there, I just tried to assign the ads module to 'sidebar-r' position and it's showing now.
The same with 'Download Your Information' page.
