my contact us page is not workiing.....

i am getting and 404 error saying the page is not found

also how do i put same link on two menus... for example if i wanto put the contact us link on top menu and bottom menu...do i have to create link on both menus...
because when i last tried to create two seperate links on both menus.. they were directed to two different contact pagess...the itemid's were different
