can't use secure image

What do I need to add to which file to get a form input for the secure image feature. The component is installed, but when I enable it in JA Submit, I get a submission form with nowhere to put the letters.

I use Security Image v3.0.0 rc2 and JA Submit v2.1... But I'm not see security image...
